Ear Hygiene Clinic Privacy Policy – Updated September 2021

We collect personal information from you, including information about your:

• Name
• Contact information
• Date of birth
• Relevant health information
• General Practitioner details
• ACC claim number (if relevant)
• War Pension number (if relevant)

We collect your personal information in order to: 

• Provide ear care related services.
• Record consent to treatment and understanding of potential side effects.
• Communicate information about appointments or other information relevant to ear care and our business operations.
• Communicate relevant healthcare information to your other relevant healthcare providers.
• Follow NZ contact tracing COVID-19 requirements if the client prefers manual contact tracing over scanning the QR Code.

Besides our staff, we may share this information with:

• Your General Practitioner and/or ENT Specialist in order to refer back/onwards for any additional treatments.
• ACC and/or Veterans Affairs in order to obtain payment when pre-organised, providing a subsidy to the client.
• Government Contact Tracers if required due to COVID-19.

Providing some information is optional.

• If you choose not to enter contact details, we'll be unable to provide reminders for further treatment when you are due for your next appointment.
• If you choose not to provide General Practitioner details, we'll be unable to provide your GP with relevant information for further treatment if required.
• If you choose not to provide your ACC claim number for long term hearing loss or your War Pension number we’ll be unable to subsidise the cost of your appointment.

The information we do require to provide ear care is your name on the signed Patient Health Questionnaire for new clients, declaring that you have read and understood the side effects of microsuctioning.
 
We keep your information safe by storing it in a secure database.
